ROCKALL AND BLATCHFORD SCORING SYSTEMS IN PROGNOSIS OF PATIENTS WITH GASTROINTESTINAL BLEEDING DUE TO PEPTIC ULCER DISEASE

Mở đầu: Hiện nay, thang điểm Rockall và Blatchford được áp dụng rộng rãi nhất để tiên đoán kết cục lâm sàng ở bệnh nhân (BN) xuất huyết tiêu hóa (XHTH) nhằm phân tầng nguy cơ ngay khi tiếp nhận ban đầu. Mục tiêu: Xác định giá trị dự đoán của thang điểm Rockall và Blatchford về nguy cơ tái xuất huyết, tử vong và yêu cầu can thiệp y khoa (CTYK) như truyền máu, nội soi can thiệp, phẫu thuật. Phương pháp: Một nghiên cứu đoàn hệ tiền cứu, khảo sát 257 BN bị XHTH do loét dạ dày tá tràng (LDDTT) điều trị tại khoa Nội Tiêu hóa, Bệnh viện Nhân dân 115 từ 09/2009 đến 06/2010. Điểm số Rockall, Blatchford và Bảng đánh giá mức độ máu mất (MĐMM) được tính ở các BN XHTH do LDDTT để xác định giá trị dự báo. Kết quả: tuổi trung bình: 59,12 ± 19,23, tỷ lệ nam/nữ: l/2, tỷ lệ tái xuất huyết: 14,79%, tỷ lệ tử vong trong bệnh viện: 2,33%, yêu cầu CTYK (truyền máu 45,14%, can thiệp nội soi 29,96%, phẫu thuật 1,95%). Điểm Blatchford trung bình 11,95 ± 3,83. Với Blatchford ≤ 6, không có trường hợp CTYK, tái xuất huyết cũng như tử vong (độ nhạy 100%, độ đặc hiệu 15,67%). Điểm Rockall toàn bộ trung bình: 3,8 ± 2,19. Tại giá trị cắt ≤ 5, được xem là nguy cơ tái xuất huyết thấp (độ nhạy: 83,78%, độ đặc hiệu: 67,73%). Điểm Rockall lâm sàng trung bình 2,28 ± 1,64. Tại giá trị cắt là 4, (độ nhạy 83,33%, độ đặc hiệu 74,10%) cho kết cục tử vong. Kết luận: Bảng điểm Rockall lâm sàng và Blatchford có ý nghĩa trong tiên đoán yêu cầu CTYK và nguy cơ tử vong. Bảng điểm Rockall toàn bộ có giá trị tiên đoán nguy cơ tử vong và tái xuất huyết. Nên áp dụng bảng điểm Rockall và Blatchford trong thực hành lâm sàng để đánh giá nguy cơ ở BN XHTH do LDDTT.

Abstract

Background: Nowadays, Rockall and Blatchford scoring systems are used worldwide for predicting the clinical outcome in the patients with gastrointestinal bleeding (GIB) so that patients may be risk-stratified appropriately. Objective: To determine predictive values of Rockall and Blatchford scoring system in estimating re-bleeding risk, mortality rate and requirement of medical interventions (transfusion, therapeutic endoscopy, surgery). Method: A prospective cohort study investigating 257 patients with upper GIB due to peptic ulcer disease (PUD) at admission was performed at 115 People Hospital ranging from 09/2009 to 06/2010. The Rockall score, the Blatchford score and the Current blood loss estimating system were calculated to determine the predict value. Results: A mean age of 59.12±3.83, male/female ratio of 1/2,09. The re-bleeding and mortality rate was 14.79% and 2.33%, respectively. The incidence of medical interventions was as follow: transfusion 45.14%, endoscopic therapy 29.96%, and surgery 1.95%. Blatchford system yielded the mean score of 11.95±3.83 with no recorded case with score less than 6 (the sensitivity and specificity were 100% and 15.67%, respectively). Mean score estimated by Rockall system was 3.8±2.19. Scores less than 5 were considered low-risk for re-bleeding (83.78% sensitive and 67.73% specific).Clinical Rockall scores yielded the mean of 2.28±1.64. At cut-point of 4, these values were 83.33% and 74.10%, respectively. Conclusion: Rockall and Blatchford scoring systems yielded significant results in predicting medical intervention requirement and mortality. Full Rockall system was valuable in estimating mortality and risk of re-bleeding. We recommend that Rockall and Blatchford scoring system should be applied in clinical practice to estimate risks in patients with upper GIB due to PUD.
