Abstract
Background: Epidural anesthesia by Bupivacaine and Fentanyl combined is an efficient way of the labor analgesia but many still question the need for pain relief during labor and delivery. This study is performed to look for the effects of epidural analgesia with Bupivacaine and Fentanyl during labor and delivery. Methods: Prospective study on 340 pregnant women in labor stage. All of them have undergone an epidural analgesia with Bupivacaine and Fentanyl. Pulse, blood presure, resspiratory rate of the pregnants and fetal heart rate of neonates were monitored right before and after analgesic injection. To determine the proportion of complications. Dose of Bupivacaine and Fentanyl, health status of pregnant women and neonates, status of labor and delivery were monitored during follow-up period. Results: The proportion of complications: hypotension: 2.6%, headache: 4.7%, backache: 13.2%, shivering: 5.3%, nausea and vomiting: 3.8%, urinary retention: 8.5%. Side-effects, accidents and complications were not noted. Fetal heart rate decreased significantly within first 30 minutes after analgesic injection. Apgar score less than 7 at 1 minute (p< 0,05) and returned gradually at the baseline level. Conclusions: Pain relief in labor by epidural anesthesia with Bupivacaine and Fentanyl is safe and effective. To decrease the disadavantages of the epidural anesthesia in the labor, we must collaborate with the obstetricians about the best methods on the basis of performing the stimulation at the approriate time, monitoring of fetal heard rates and specially the intervention must be carried down at time .
