Abstract
Dexmedetomidine is a highly specific, potent, and selective 2 adrenoreceptor agonist. It has a relatively high ratio of 2/1 activity (1620:1 as compared to 220:1 for clinidine). 2 adrenergic agonists decrease sympathetic tone with ensuing attenuation of neuroendocrine and hemodynamic responses to stress, reduce anesthetic and opioid requirement and analgesic effect. The aim of our study is evaluate efficacy of perioperative Dexmedetomidine infusion on hemodynamic effect, economize anesthetic and opioid required during operation as well as sedative -analgesia effects in postoperative in patient undergoing coronary artety bypass grafting. Methods: 40 patients scheduled for coronary artery bypass grafting (CABG) received a dexmedetomidine intravenous infusion beginning 10 minutes before induction of anesthesia and continuing until 24 hour after the end of surgery. All patients received intravenous anesthesia with TCI method. Heart rate and blood pressure were kept within predetermined limits by varying anesthetic level and using vasoactive medications. Heart rate, blood pressure, cardiac output, intravenous anesthetic concentration were monitored continuously during operation. Sedation level and pain score were assessed by the managing clinical team on duty based on anagesia assessment performed with the VAS score (1 - 10 points) every hourResults: Our study includes 13 females and 27 males, the mean of age (years) is 61. 82 ± 9.31 (30 - 75); the mean of extubation time (hour) is 10.05 ± 2.01. During operation the reduction of total dose of sufentanil and propofol is 24.8% and 42.85%. Hemodynamic is stable during per and postoperative. Conclusion: the use of Demedetomidine as an anesthetic adjunct in the patients undergoing CABG has significant advantages: maintaining stable hemodynamic during operation, economizing the anesthetic agents and early extubation.
