ADHERENCE TO MEDICATION, LIFESTYLE MODIFICATIONS IN PATIENTS WITH HYPERTENSION IN NHAN DAN GIA DINH HOSPITAL

Đặt vấn đề: Khuyến cáo mới nhất về tăng huyết áp của Hiệp hội tim mạch châu Âu nhấn mạnh tầm quan trọng của tuân thủ lên hiệu quả kiểm soát huyết áp. Tuân thủ điều trị làm giảm nguy cơ biến cố tim mạch và giảm tỉ lệ tử vong ở mọi độ tuổi.

Mục tiêu: Xác định tỉ lệ tuân thủ dùng thuốc, tuân thủ lối sống, các yếu tố liên quan đến sự tuân thủ; mối liên quan giữa tuân thủ điều trị và kiểm soát huyết áp.

Phương pháp nghiên cứu: Mô tả cắt ngang. Tuân thủ dùng thuốc đánh giá qua thang đo MMAS-8. Đánh giá tuân thủ lối sống qua hành vi: giảm mặn, ngưng hút thuốc lá, bia rượu, tập thể dục.

Kết quả: nghiên cứu gồm 243 bệnh nhân, tuổi trung bình 63,6 ± 8,2, nữ chiếm 59,7%. Tỉ lệ tuân thủ dùng thuốc, tuân thủ lối sống và tuân thủ điều trị chung lần lượt là 78,6%, 51,9% và 45,3%. Trình độ học vấn cao hơn có liên quan đến tuân thủ dùng thuốc (OR = 2,515; CI 95%: 1,006 – 6,332; p=0,049). Nam có tỉ lệ tuân thủ lối sống thấp hơn nữ (OR = 0,389; CI 95%: 0,222 – 0,681; p=0,001). Có sự liên quan có ý nghĩa thống kê giữa việc tuân thủ điều trị chung và hiệu quả kiểm soát huyết áp (OR = 3,250; CI 95%: 1,485 – 7,112; p=0,003).

Kết luận: Tỉ lệ tuân thủ điều trị chung còn thấp. Bệnh nhân có trình độ học vấn tiểu học có tỉ lệ dùng thuốc thấp hơn. Bệnh nhân nữ có tỉ lệ tuân thủ lối sống cao hơn. Kết hợp tuân thủ dùng thuốc và tuân thủ lối sống có liên quan đến tăng tỉ lệ đạt huyết áp mục tiêu.

Background: New European Hypertension Guidelines (ESC/ESH 2018) emphasized the importance of treatment adherence as a major cause of poor blood pressure control. However, previous studies have mostly focused on investigating medication adherence, while lifestyle changes are equally important in blood pressure control.

Objectives: Evaluate adherence of hypertensive patients to pharmacotherapy and non-pharmacological measures, patient-related factors associated with treatment adherence, the association between treatment adherence and blood pressure control.

Methods: A cross-sectional, descriptive study. 8-item Morisky adherence scale was used to assess medication adherence. Non-pharmacological adherence was assessed in four categories: salt restriction, smoking habit, alcohol consumption and physical exercise. Statistical analysis was accomplished by Chi-square and logistic regression analysis.

 

Results: Of 243 patients included in the study, mean age was 63.6 ± 8.2 years, 59.7% were female. Percentage of patients adhered to antihypertensive medications, lifestyle modifications were 78.6%, 51.9%% respectively. Higher education levels were associated with drug compliance (OR = 2,515; 95% CI: 1,006 - 6,332; p = 0,049). Men has lower rate of lifestyle adherence than women (OR = 0.389; 95% CI: 0.222 - 0.681; p = 0.001). There was a significant asscociation between overall treatment adherence and blood pressure control (OR = 3.250; CI 95%: 1.485 – 7.112; p = 0.003).

Conclusion: Percentage of hypertensive patients adhered to medication and lifestyle modifications was low (45.3%). Patients level education was related to medication adherence. Gender was related to healthy lifestyle adherence. Overall treatment adherence was associated with successful blood pressure control.
