Mục tiêu: Đánh giá kết quả phẫu thuật Furlow điều trị thiểu
sản vòm trên bệnh nhận di chứng khe hở vòm.
Phương pháp nghiên cứu: Có 32 trẻ chẩn đoán thiểu sản vòm hầu được điều
trị bằng phẫu thuật Furlow tại Khoa Sọ mặt và Tạo hình, bệnh viện Nhi Trung
Ương từ 2015-2017. Phương pháp nghiên cứu mô tả.
Kết quả: Gồm 15 trẻ nam chiếm 46,87%, 17 trẻ nữ 53,13%.
Tuổi can thiệp trung bình 5,72 ± 2,34, từ 3 đến 8 tuổi. Tình trạng thoát khí
qua đường mũi khi trẻ nói gặp 100% trẻ trước can thiệp, sau can thiệp còn lại 5
trẻ (15,62%). Khả năng đóng kín vòm trước phẫu thuật ở độ II, độ III lần lượt
là 13 trẻ (40,61%), 19 trẻ (59,37%), sau phẫu thuật 30/32 trẻ đóng kín vòm gần
như hoàn toàn (độ I), còn lại 2 trẻ (6,25%) ở độ II. Nói ngọng phụ âm cũng cải
thiện, trước can thiệp gặp 100% trẻ, sau can thiệp gặp 5 trẻ (15,62%). Không có
trường hợp tử vong, không ghi nhận thông mũi miệng sau phẫu thuật.
Kết luận: Phẫu thuật Furlow điều trị thiểu sản vòm có kết
quả bước đầu tốt, an toàn, khả thi và có thể áp dụng được tại các bệnh viện lớn.

Objectives: This study aimed to assess the outcomes of using
Furlow surgery in the treatment of velopharyngeal insufficiency (VPI) among
patients with postoperative palatoplasty.
Methods: This study consisted of 32 childrens diagnosed
with velopharyngeal insufficiency who were treated using Furlow surgery between
2015 and 2017 at the Department of Craniofacial and Plastic Surgery, Vietnam’s
National Hospital of Children. This study is a descriptive study.
Results: The study population included 15 (46.87%) male
and 17 (53.13%) female children. The average age at surgical intervention was
5.72 ± 2.34 years (range, 3-8 years). Air flowing through the nose when
speaking was seen in all patients before surgery, and was seen in 5 (15.62%)
patients after surgery. As assessed preoperatively, the possibility for
complete closure of the VP level II and III was 13 (40.61%) and 19 (59.37%)
patients, respectively. Postoperatively, almost complete closure of the
velopharynx was achieved for 30/32 patients (level I), and was not achieved for
2 (6.25%) patients (level II). Hypernasal resonance with consonant also
improved; all patients had this condition before surgery as compared to only 5
(15.62%) patients had this condition after surgery. No case of death and
nasal-oral fistula was observed after surgery.
Conclusions: The use of Furlow surgery in treating VPI was
shown to be effective, safe, and have to potential to be performed in large hospitals.
