Background: In immune thrombocytopenia (ITP) and autoimmune
hemolytic anemia (AHA) diseases, corticosteroids are the first of choice in
treatment regimens and the treatment usually last for many months or years. In
fact the osteoporosis screening for those patients have not been applied
systematically, the bone mineral density, the proportion of osteoporosis in
this patients has not been studied in ChoRay hospital.
Objective: Determine the ratio of the group taking
corticosteroids osteoporosis at doses ≥ 5 mg/day for ≥ 3 months time compared
with no treatment.
Subjects and method: Study design: cross sectional study 114 patients
(92 ITP, 22AHA) divided into 2 groups: 84 patients were treated corticosteroids
during ≥ 3 months and a control group of 30 patients with no corticosteroids
treatment. Measure BMD at the lumbar spine and femoral neck by the dual energy
X-ray absorptiometry (DXA) method with
diagnostic criteria for osteoporosis is the T-score ≤ -2.5.
Result: 29.8% of patients with ITP and AHA
usingcorticosteroids for long time were suffered from osteoporosis. 3.3% of
patients not using corticosteroids were osteoporosis.
The usagetime of
corticosteroids for more than 3 months; BMI; and age were factors affecting
osteoporosis (p <0.05). Menopause status could increase the risk of
osteoporosis in women who takecorticosteroids. However, this has no effects to
statistical data.
Conclusions: The osteoporosis rate of patients who have usedfor
long time will be higher than those who have never use corticosteroids. The
following factors: elderly patients, lower BMI, menopausal status, will
contribute to the increase of osteoporosis rate of patients
takingcorticosteroids in long period.
Keywords: Bone mineral density, osteoporosis,
corticosteroids, immune thrombocytopenia (ITP), autoimmune hemolytic anemia
(AHA).
