freundii (0,7%), Enterobacter cloacae (0,7%), P. aeruginosa (0,7%).
Introduction: Intra-abdominal infections (IAIs) is a common infection in surgery; is one of dangerous acute diseases, infections
can spread and cause mortality. The care and
treatment should be considered because the causative factors may be very
different, depending on the involved organ systems. The rate of infection after surgery is also
different due to the combination with the invasiveness of surgical procedures,
and other factors such as age and medical conditions of the patient. The use of inappropriate
cephalosporin in initial preventive treatment of surgical departments as well as in agent treatment of the intensive care units to IAIs likely cause
expansion of the extended spectrum beta
–lactamase (ESBL). This was observed in
several countries, including Vietnam.
Objective: To investigate
the distribution of the bacteria kinds
that causes common IAIs and the tendency of their
antibiotic resistance. To monitor continuously the antibiotic
sensitivity of Gram-negative bacteria that causes IAIs. To compare
the
sensitivity of Ertapenem and other
antibiotics.
Methods: Prospective study. This is a multi-center epidemiological study of nearly 120 countries in the world, Vietnam has
four centers: Cho Ray Hospital, Binh Dan Hospital, Vietnam-German Hospital and
Bach Mai Hospital. Selected samples are pus, fluid from IAIs that collected in surgical process. Data of isolated bacteria and antibiotic sensitivity from samples of IAIs were collected from
Cho Ray Hospital from 2011-2013. Data were analyzed by IHMA (USA).
Results: Isolation of 289 bacteria from patients with
IAIs. These are common bacteria: E. coli (81%) ESBL (48.7%), Klebsiella spp. (12.1%) ESBL (37.1%), A. baumannii (1.7%), Proteus
mirabilis (1.7%), Citrobacter
freundii (0.7%), Enterobacter cloacae (0.7%), P. aeruginosa (0.7%). There are differences between the levels of
drug-resistant in bacteria. The intestinal bacterium resists many antibiotics, however there is a low level of carbapenem resistance.
Conclusion: The bacteria causing IAIS is mainly E. coli (81%),
K. pneumoniae (12.1%). The antibiotics are sensitive to E. coli: ertapenem,
Imipenem, Amikacin >90%; Piperacillin / Tazobactam >80%. For Kleb
bacteria. Pneumonia, the sensitivity varies by year, with the decreased
susceptibility> 20% with ertapenem and other antibiotics.
