Đặt vấn đề U nội tiết tụy tiết ra insulin (insulinoma) rất hiếm gặp, tỷ lệ khoảng
5/1.000.000, có biểu hiện nổi bật nhất là hạ đường huyết, khó chẩn đoán vì đa số
u<2cm.
tuyến tụy (nesidioblastosis) phù hợp với hạ đường huyết do tăng sinh tế bào tuyến
tụy không có u (NIPHS).

Background: Insulin- secreting pancreatic tumor (insulinoma)
was very rare - incidence of 5/1,000,000 with obvious symptoms of hypoglycemia,
posing difficulty in diagnosis due to its tiny diameter (most were <2cm).
Even more difficult scenario encountered when a patient with obvious
hypoglycemic episodes coincided with elevation of C-peptide and insulin level
however no pancreatic tumor detected on CT scan or MRI.
Methods: Retrospective analzysed all hypoglycemia patient with or without tumor of
the pancreas that were operated at the Hepato-biliary-pancreatic department of
Cho Ray hospital during the period of May 2005 and October 2015.
Results: There were 30 hypoglycemic patients, with 28
insulinoma, 1 neuro-endocrine tumor, 1 hyperplasia of islet tissue without
tumor, operated at HBP department of Cho ray hospital. There were 9 male (30%,
21 female (70%) with mean age of 45,13±2. Surgical interventions included
simple tumor excision 12 cases (40%), tumor excision plus pancreatic duct-
jejunal anastomosis 3 cases (10%), distal pancreatectomy 13 cases (43,4%),
pancretoduodenectomy 1 case (3%), excision of tumor plus duodenectomy 1 case
(3%). One intra-operative bleeding at the tail of pancreas (3.3%) that was
successfully coagulated, three pancreatic fistula (10%) that healed
spontaneously. There were no mortality. All hypoglycemia regained normal
glycemic level right after tumor removal or resection of pancreas.
Conclusions:
Surgical treatment had successful
rate as high as 100% with all patients regained normal glycemic level during
hospital stay as well as long term follow-up. Operative incidence and
complications were low, with no mortality. In very rare case where all imaging
techniques failed to discover tumor then surgeon clinical experience was of
utmost importance.. Anatomo- pathology showing hyperplasia of pancreatic islet
tissue without tumor (nesidioblastosis) confirming diagnosis of NIPHS (Non
Insulinoma Pancreatogenous Hypoglycemia Syndrome).
