Mục đích: Xác định tính khả thi và an toàn của phương pháp
nuôi ăn sớm qua đường miệng sau phẫu thuật cắt dạ dày do ung
thư.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu, thử nghiệm lâm sàng không nhóm chứng trên 50 bệnh nhân
(BN) phẫu thuật cắt dạ dày do ung thư tại khoa Ngoại tiêu hóa bệnh viện Chợ Rẫy.
thi và an toàn.
Aims: To evaluate the feasibility and safety of early
oral feeding after gastrectomy for gastric cancer.
Methods: Prospective study, uncontrolled clinical trial.
We studied 50 patients with gastrectomy due to gastric cancer in the Digestive
surgery Department at Cho Ray Hospital. Nasogastric tubes were withdrawn within
12-24 hours postoperatively, Patients were then orally fed with Glucose 5% on
postoperative day (POD) 1-2 and then liquid to special porridgefrom POD 3 until
discharge.
Results: From 11/2014 to 6/2015, we included 50 patients
into study, in which, 40 males and 10 females with mean age of 55. Thirty five
patients (70%) underwent distal gastrectomy and 15 (30%) underwent total
gastrectomy. There was no mortality and no postoperative complications. Eight
patients (16%) had gastrointestinal disorders, in which 5 patients had mild
abdominal distention who can continue oral feeding, 3 patients (1 diarrhea, 1
severe abdominal distension, 1 nausea) who can not continue oral feeding and
maintain intravenous feed. The mean times of nasogastric tube withdrawal were
22.8 ± 2.8 h and Glucose 5% intake were 24.1± 2.9 h. Mean times of having
flatus were 48.7 ± 5.5 h and postoperative hospital staywere 6.7 ± 0.7 days.We
also noticed increasingof blood lymphocytes, serum prealbumin after early oral
feeding intervention.
Conclusions: Postoperative early oral feeding after
gastrectomy for gastric cancer is feasible and safe. Blood lymphocytes, serum
prealbumin were increased after early oral feeding intervention. A randomized
controlled trial should be performed to better evaluate the role of
postoperative early oral feeding after gastrectomy for gastric cancer.
Keywords: Early oral feeding, postoperative, gastrectomy.
