thực trạng viêm phổi do nấm hay nhiễm nấm xâm lấn ở phổi nhằm cải thiện các biện
pháp chẩn đoán và điều
trị trong tương lai.
Phương pháp: Thiết kế nghiên cứu hồi cứu
mô tả, thực hiện trên 47 trường hợp chẩn đoán
viêm phổi do nấm hoặc nhiễm nấm đường hô hấp trong 2 năm 2012 –
trường hợp chẩn đoán nhiễm nấm phổi do

Aim of study: To evaluate the current situation of fungal
pneumonia or invasive fungal infections of the lungs in order to improve the
diagnosis condition and antifungal chemotherapy in future.
Methods: A retrospective study design was conducted on 47
patients which were diagnosed fungal pneumonia or invasive fungal infection of
the respiratory tract in 2 years 2012 – 2013 at Choray hospital.
Results: The results showed that males encountered 34
cases (72.3%) and females 13 cases (27.7%). The mean age 54 (22-77). 23 of 47
cases (49%) with the final diagnosis fungal infection of the sinus which were
excluded from the data analysis. About half of patients had three-week duration
of the present illness history before admission. Twenty-four of forty-seven
cases (51%) which were diagnosed fungal pneumonia with fulfilled clinical
criteria, microbiology and imaging diagnosis. The predisposing factors of
fungal pneumonia are commonly seen as pulmonary tuberculosis, COPD, pulmonary
tumors, bronchiectasis, diabetes, corticosteroids and long-term therapy with
broad spectrum antibiotics. The symptoms are usually seen as fever, hemoptysis,
and cough and chest pain. Twenty-four fungal infections cases were mainly due
to Candida sp. and Aspergillus sp. The others endemic or opportunistic fungal
pathogens were not detected in this study. Sixteen of twenty-four cases (66.66%)
were identified with Candida albicansor, Candida kruseiin sputum or
broncho-alveolar lavages. Eight of twenty-four cases (33.33%) were diagnosed
invasive fungal infection with Aspergillus sp. based on the results of imaging
diagnosis and/or histopathology by biopsy or confirmed after surgical
interventions. Only two of eight cases were identified as Aspergillus sp. by
broncho-alveolar lavages. All the fungal pneumonia with invasive aspergillosis was
treated with pulmonary tumor resection and then was continued with antifungal therapy.
There was one case refused surgical intervention and one dead case due to
invasive candida infection. For Candida sp. infections, most of cases were
resistant to fluconazole and itraconazole, susceptible only to amphotericin B.
Conclusions: The diagnoses were quite late in pulmonary
fungal tumors; most of cases were diagnosed by imaging and confirmed by
histopathological results after surgery. The results of this study suggested enhancing
and improving the facilities in order to have early diagnosis for invasive
fungal infections. Most of isolates of invasive candidiasis and aspergillosis
identified in this study were resistant to all available antifungals as
fluconazole and itraconazole even with amphotericin B; therefore, it is
necessary to introduce early new generation’s antifungals in order to have the
better treatment outcomes.
