Objectives: To clinically assess and categorize traumatic
injuries of the upper eyelids involving loss of tissues. To indicate
appropriate management and evaluate patients’ functional and aesthetic outcomes
Patients and Methods: The study is designed as a prospective series
enrolling patients with traumatic injuries of the upper eyelids involving
tissue loss who were admitted to the Department of Ophthalmology at
Results: Traumatic injuries of the upper eyelids involving
tissue loss can occur in patients of all ages, most commonly among those >15
to 50 years of age (73.4%). Men accounted for 88.2% of the patients. The most
common cause was traffic accidents. Administration within 24 hours of injury
was noted in 85.3% of all patients. Loss of < 25% of the eyelid tissues
occurred in 61.8% of the patients. Injuries anterior to the orbital septum were
observed in 70.6% of the cases, whereas complex injuries were in 47.1%. Functional
recovery was achieved in 73.5% of the cases. The rate of complete recovery
after treatment was 47.1%; that of minor scarring contracture not causing lid
retraction, ptosis was 26.5%, and the rate of lid retraction, ptosis grade 3–4
was 5.9%. Patient satisfaction after treatment was 64.7%, and the acceptable
rate was 32.4%.
Conclusions: Traumatic injuries of the upper eyelids involving
tissue lossappeared to be very diverse, often associated with other injuries.
In most cases, the area of the lost tissues were < 25% and the injuries
occurredanterior to the orbital septum. Clinical management required a complete
assessment of all injuries, particularly those potentially affecting patients’
life and the eyeballs. Immediate application of cold saline and administration
of antibiotics should be included. The anatomical, functional, and aesthetic
outcomes were satisfactory in cases with tissue loss of less than 60% of the
eyelid area.
