Mục tiêu: Đánh giá hiệu quả phương pháp tiêm mỡ tự thân vào dây thanh qua nội soi điều trị hở thanh môn do teo dây thanh, khuyết lõm dây thanh.

Phương pháp-đối tượng: Nghiên cứu loạt ca, có can thiệp, từ 2010 đến 2015, 25 bệnh nhân hở thanh môn (14 teo dây thanh, 11 khuyết lõm dây thanh) được tiêm mỡ tự thân qua nội soi. Đánh giá hiệu quả sau tiêm mỡ 1, 3 và 6 tháng bằng các chỉ số phân tích âm, chỉ số khuyết tật giọng nói, soi hoạt nghiệm thanh quản và mức độ hài lòng của người bệnh.

Kết quả: Sau tiêm mỡ, tỉ lệ bệnh nhân cải thiện tốt đạt 68,2%, có cải thiện đạt 20%, không cải thiện là 11,8%. Trong đó cải thiện rối loạn các chỉ số : Phân tích âm (67,7%), sóng rung niêm mạc (54,9%), độ khép kín thanh môn (81,8%), VHI (86,5%), tự cảm nhận (63,6%). Không có biến chứng xảy ra.

Kết luận: Tiêm mỡ tự thân điều trị hở thanh môn do nhóm bệnh lý teo dây thanh và khuyết lõm dây thanh hiệu quả cao, sinh lý, an toàn.

Abstract

Objective: Evaluating the effectiveness of autogenous fat injection (AFI) with endoscopy for glottic insufficiency due to Vocal Cord Atrophy (VCA) and Vocal Cord Sulcus (VCS).

Methods: Case series and intervention study over the period from 2010 to 2015, a total of 25 patients who suffered from glottic insufficiency (14 patients with VCA, 11 patients with VCS) were injected autogenous fat with rigid endoscopy procedure. The effectiveness of AFI after 1, 3 and 6 months was evaluated with acoustic indexes analysis, voice handicap index, videolaryngostroboscopic and level of patient’s satisfaction.

Results: After procedure, the percentage of patients excellently improved accounts for 68.2%, those improved makes up 20% and those show no improvement represents 11.8%. Improves disorder indicators: Analysis of acoustic indexes (67.7%), mucosal wave vibration (54.9%), complete glottal closure (81.8%), VHI (86.5%), patient self-assessments (63.6%). No complications were observed.

Conclusion: The autogenous fat injection with endoscopy for glottic insufficiency due to VCA and VCS is a highly effective, physiological and safe procedure.
