Mục tiêu: Đánh giá hiệu quả bước đầu của máy cắt đốt tần số radio
trong phẫu thuật nội soi cắt dây thanh trên bệnh nhân liệt cơ mở thanh quản hai
bên.
Phương pháp nghiên cứu:
Thiết kế nghiên cứu mô tả ca bệnh
có can thiệp lâm sàng.
tần số radio rất an toàn, dễ thực hiện, nhanh gọn, ít xâm lấn, ít biến chứng,
Purpose: To evaluate the results of endoscopic
posterior cordotomy using microdissection radiofrequency electrodes in patients
with bilateral abductor vocal cord paralysis.
Method: This prospective experimental study included 26
patients with bilateral abductor vocal cord paralysis, which came to our
hospital from Jan 2014 to Sep 2014. All patients, endoscopic posterior
cordotomy was done with radiofrequency Arrowtip electrodes under general
anesthesia. These patients were followed up for 3 months.
Results: The most common cause was paralysis the
recurrent laryngeal nerve after thyroidectomy (22 patients, 84.6%) with all of
them are female. Group of age ranging from 50-70 occupied 80.7%. Preoperative
severe clinical symptoms were dyspnea (77%), snoring (100%). Mean operation
time was 17 minutes. Postoperatively, decannulation was successful 100% with
average duration about 2 days, 100% patients without snoring. The mean
discharge time was 3 days. After
following surgery 3 months, 92.3% patients had understandable voice and 100%
patients had mild affected voice with VHI scoring < 60. Spirometry was good:
61.6% after one week, 96.2% after one month and 100% after three months. There
was no immediate complication. Two patients required a secondary revision
operation due to granulation formation after surgery one month.
Conclusion: The data indicated the safety, minimally
invasive, easy use, and efficiency of the microdissection radiofrequency electrodes
in patients with bilateral vocal fold paralysis. This technic provides a
reliable procedure to solve the problem of difficult airway in patients with
bilateral vocal cord paralysis without adversely affecting voice quality of
patient.
