Mục tiêu: Mục tiêu của báo cáo này là đưa ra một số nhận xét ban đầu về phẫu thuật nối bắc cầu bạch mạch - tiểu tĩnh mạch trong điều trị phù bạch mạch của chi trên ở bệnh nhân ung thư vú.
Đối tượng và phương pháp: Gồm 3 bệnh nhân phù bạch mạch chi trên sau điều trị ung thư vú được phẫu thuật nối bắc cầu bạch mạch - tiểu tĩnh mạch bằng kỹ thuật “siêu vi phẫu”.
Kết quả: Số miệng nối trung bình là 3,3 (2-5 miệng nối), kích thước của bạch mạch từ 0,3 - 0,8 mm. Thời gian phẫu thuật trung bình là 4,5 giờ (3,5 - 5 giờ). Tất cả bệnh nhân xuất viện dưới 12 giờ sau phẫu thuật. Cả 3 bệnh nhân đều cảm thấy cải thiện ngay sau phẫu thuật. Sự khác biệt về chu vi của tay phù sau mổ 1 tháng giảm 20% và 3 tháng giảm 25%. Không có trường hợp nào ghi nhận biến chứng sau phẫu thuật hoặc tình trạng phù gia tăng.
Kết luận: Phẫu thuật nối bắc cầu bạch mạch - tiểu tĩnh mạch có thể làm giảm một cách hiệu quả mực độ trầm trọng của phù bạch mạch ở bệnh nhân ung thư vú. Tuy nhiên cần được đánh giá với số lượng bệnh nhân nhiều hơn và theo dõi lâu dài hơn.

IN BREAST CANCER PATIENTS: SOME PRELIMINARY REMARKS
Background - objectives: The treatment of lymphedema is difficult and challenging problem. Lymphatico-venous anastomosis (LVA) is widely used as surgical treatment for secondary lymphedema, with resolution of lymph retention by formation of a venous-lymphatic bypass.The purpose ofthis report was to provide some preliminary remarks of lymphaticovenular bypass for the treatment of upper limb lymphedema in breast cancer patients.
Methods: Three patients with upper extremity lymphedema secondary to treatment of breast cancer underwent lymphaticovenular bypass using a “supermicrosurgical” approach. Evaluation included qualitative assessment and quantitative volumetric analysis before surgery and at 1 month, 3 months after the procedure.
Results: The mean number of bypasses performed per patient was 3.3 (range, two to five) and the size of bypasses ranged from 0.3 to 0.8 mm. The mean operative time was 4.5 hours (range, 3.5 to 5 hours). Hospital stay was less than 12 hours for all patients. All patients reported symptomimprovement following surgery. The mean volume differential reduction was 20 percent at 1 month, 25 percent at 3 months. No patients experienced postoperative complications or lymphedema exacerbation.
Conclusions: Lymphaticovenular bypass may effectively reduce the severity of lymphedema in breast cancer patients. More patients need to be evaluated and long-term analysis is needed.
