APPLICATION OF ENDOSCOPE - MINIMAL ACCESS TECHNIQUES IN THE MANAGEMENT OF FRONTAL SINUS FRACTURES : 64 CASES

Mục tiêu: Các phương pháp tiếp cận kinh điển trong xử trí vỡ xoang trán qua đường rạch liên thái dương hình vòng cung, đường nửa vòng cung, đường Jacques, đường Sebileau Lothrop.
Các phương pháp tiếp cận này mang lại sự bộc lộ tuyệt vời đối với xoang trán, tuy nhiên chúng đòi hỏi phải thực hiện đường rạch da rộng, và có thể để lại sẹo thậm chí hói đầu. Các tác giả giới thiệu kỹ thuật sử dụng ống nội soi trong điều trị chấn thương vỡ xoang trán.
Thiết kế nghiên cứu: một hoặc 2 đường rạch da 1,5cm trên cung mày, qua đó đưa ống nội soi vào.
Abstract
Objectives: Classic approaches to frontal sinus fractures involve bicoronal incision, hemicoronal incision, Jacques incision, Sebileau Lothrop incision. These approaches afford good exposure to the entire frontal sinus : however, they require a large external incision, and they cause scarring even alopecia. The authors present their technique with the repair of frontal sinus fractures using the endoscope.
Study design: One or two 1,5cm incisions were placed in the eyebrow, through which a 4mm 0 , 30 endoscope were inserted. The depressed fractures segments of the anterior table were reduced without fixation. Result: 64 consecutive patients received endoscopic correction of frontal sinus fractures from2002 to 2005 at Cho Ray hospital. The most commom cause of injury was traffic accident. Computed tomography and 3 D reconstructions had been done before surgery. No patient required conversion conventional bicoronal incisions. Good anatomic reduction of the fractures sites, small acceptable sugical scar. The postoperative course was uneventful, without any complications.
Conclusion: using endoscope is very useful and successful in treatment of frontal sinus trauma. The method has some limitations in displaced posterior fracture with CSF leaks. In these cases, the best method is cranialized...
