Mục tiêu: Suy giảm nhận thức là biến chứng thường gặp sau nhồi máu não, liên quan đến tàn tật lâu dài, tuy nhiên dữ liệu theo dõi tiến triển tại Việt Nam còn hạn chế.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu đoàn hệ tiến cứu trên bệnh nhân nhồi máu não lần đầu mức độ nhẹ – trung bình (NIHSS <15). Chức năng nhận thức được đánh giá bằng thang điểm MoCA tại 3 và 6 tháng.
Kết quả: Trong 98 bệnh nhân, tỷ lệ suy giảm nhận thức giảm từ 62,2% tại 3 tháng xuống 45,9% tại 6 tháng; điểm MoCA trung bình tăng có ý nghĩa từ 19,98 ± 7,97 lên 21,78 ± 6,98 (p <0,001). Trình độ học vấn thấp, thể tích nhồi máu lớn và gánh nặng tổn thương chất trắng – teo não là các yếu tố liên quan có ý nghĩa với suy giảm nhận thức tại cả hai thời điểm (p <0,05). Chức năng điều hành và trí nhớ trì hoãn bị ảnh hưởng nhiều nhất nhưng cũng cải thiện rõ nhất theo thời gian.
Kết luận: Suy giảm nhận thức sau nhồi máu não phổ biến nhưng có xu hướng cải thiện trong 6 tháng đầu. Cần sàng lọc và theo dõi định kỳ để định hướng can thiệp phục hồi.

Objectives: Post-stroke cognitive impairment is common and associated with long-term disability, yet longitudinal data in Vietnamese populations remain limited.
Methods: We conducted a prospective cohort study of patients with first-ever mild-to-moderate ischemic stroke (NIHSS <15). Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA) at 3 and 6 months.
Results: Among 98 patients, the prevalence of cognitive impairment decreased from 62.2% at 3 months to 45.9% at 6 months; mean MoCA scores improved significantly from 19.98 ± 7.97 to 21.78 ± 6.98 (p <0.001). Lower educational level, larger infarct volume, and higher burden of white matter changes and brain atrophy were significantly associated with cognitive impairment at both time points (p <0.05). Executive function and delayed recall were the most affected domains but also showed the greatest rate of recovery over time.
Conclusions: Cognitive impairment is frequent after ischemic stroke but shows partial recovery within the first 6 months. Routine cognitive screening and longitudinal follow-up are essential to guide rehabilitation strategies.
