Đặt vấn đề: Hội chứng thận
hư (HCTH) ở trẻ em có thể thứ phát sau nhiễm HBV mạn. Corticoid cải thiện các
triệu chứng của HCTH nhưng làm tăng sự sao chép, chậm quá trình đào thải siêu
vi và suy gan cấp do sự tái hoạt của hệ miễn dịch khi ngưng thuốc đột ngột. Chúng
tôi báo cáo một trường hợp HCTH thứ phát do HBV được điều trị thành công bằng
thuốc kháng siêu vi Tenofovir và thuốc ức chế miễn dịch Prednisone +
Cyclosporine.
Kết quả: Bệnh nhân nữ 11
tuổi, nhập viện lần đầu vào tháng 01/2015 vì phù toàn thân, tiểu vàng, HA 15/10
cmHg, albumine máu 1,77g%, cholesterol 8,88 mmol/l, creatinine cao nhất
251micromol/l, C3 37,5 mg/dl và C4 4,9 mg/dl, HBsAg (+), HBeAg(+), Anti HBc
total (+), HBV-DNA 1106x106 copies/ml, Anti HCV (-), AST 80U/l, ALT
69U/l. Tổng phân tích nước tiểu: protein 4+, Máu 3+, Protein niệu 24 giờ:
12,6g/1,73m2 da/ngày. Sinh thiết thận phù hợp với viêm cầu thận tăng
sinh màng do phức hợp miễn dịch. Điều trị năm tháng bằng Tenofovir, lợi tiểu và
hạ áp không cải thiện HCTH và cao huyết áp mặc dù giảm được HBV-DNA 1753
copies/ml và cải thiện chức năng gan (AST 19,4U/l, ALT 10,62U/l). Lui bệnh hoàn
toàn HCTH và virus học (HBeAg âm, HBV-DNA ≤ 250 copies/ml) với điều trị
Cyclosporine và Prednisone kết hợp với thuốc kháng siêu vi Tenofovir.
Kết luận: Điều trị thuốc ức
chế miễn dịch kết hợp thuốc kháng siêu vi Tenofovir có thể tạo ra sự lui bệnh
HCTH thứ phát do HBV mà không đáp ứng với thuốc kháng siêu vi đơn độc và không
gây ra sự tái hoạt của HBV và sự bùng phát của tổn thương gan.

Background: Childhood
nephrotic syndrome may be secondary from chronic HBV infection. Corticosteroid
may ameliorate symptoms of nephrotic symdrome but increase viral replication,
delay the process of viral elimination and induce an acute hepatic failure
caused by rebound reactivation of the immune system after abrupt cessation of
the drug. We report a case with HBV-related nephrotic syndrome who was
successfully treated with an anti-viral agent Tenofovir and immunosuppressive
drugs Prednisone and Cyclosporine.
Patient and method: case
report.
Result: An eleven-year-old
female patient had the first-time admission in our hospital on Januarry 2015
because of general edema, yellow urine and high blood pressure (PB 15/10 cmHg),
serum albumine 1.77g%, cholesterol 8.88 mmol/l, highest creatinine 251
micromol/l, C3 37.5 mg/dl and C4 4.9 mg/dl, HBsAg (+), HBeAg(+), Anti HBc total
(+), HBV-DNA 1106x106 copies/ml, Anti HCV (-), AST 80U/l, ALT 69U/l.
Urinary analysis was reported as protein 4+, blood 3+, 24-hour proteinuria was
12.6g/1.73m2/day. Renal biopsy was consistent with immune-complex
membranoproliferative glomerulonephritis. Five-month regimen with Tenofovir,
diuretic and anti-hypertensive drugs did not ameliorate hypertension and
nephrotic state inspite of relief of serum HBV-DNA 1753 copies/ml and
amelioration of serum hepatic enzymes (AST 19.4U/l, ALT 10.62U/l). Complete
remission of nephrosis and virology (negative serum HBeAg, HBV-DNA ≤ 250
copies/ml) was achieved with combination of immunosuppressive agents
Cyclosporine, Prednisone and anti-viral medication Tenofovir.
Conclussion: Therapeutic
regimen combined with immunosuppressive agents and anti-viral medication
Tenofovir can achive remission of HBV-related nephrotic syndrome, which did not
respond with anti-viral drug alone, without reactivation of viral replication
and acceleration of hepatic injuries.
