CORAL IMPLANTS IN THE TREATMENT OF ATROPHIC RHINITIS

Đặt vấn đề: Viêm mũi teo là một bệnh tương đối ít gặp trên thế giới nhưng cũng xuất hiện ở Việt Nam. Điều trị nội khoa có kết quả tạm thời, khi ngưng thì bệnh phát lại. qua nhiều năm nghiên cứu đã cho thấy rõ điều trị phẫu thuật góp phần làm giảm triệu chứng bệnh.

Phương pháp: 4 bệnh nhân chẩn đoán viêm mũi teo điều trị nội khoa không khỏi bệnh, được nhập viện điều trị phẫu thuật làm hẹp khoang mũi. Dùng San hô ghép vào hốc mũi hai bên của người bệnh dựa theo khí áp mũi. San hô được ghép vào dưới niêm mạc vách ngăn và sàn mũi làm giảm thể tích cửa mũi trước và sau 2 bên mũi.

Kết quả: phẫu thuật điều trị 4 bệnh nhân: ba bệnh nhân cải thiện rõ triệu chứng: không hôi, hết vảy, nhức đầu, chảy máu mũi…, một bệnh nhân giảm triệu chứng rõ nhưng không hết hẳn. Không có trường hợp nào xảy ra biến chứng: viêm nhiễm, thải ghép hay tái hấp thu của vật liệu.

Kết luận: chúng tôi thực hiện ghép san hô dưới niêm mạc vách ngăn và sàn mũi qua nội soi dưới gây mê nội khí quản là phẫu thuật khá đơn giản. Vật liệu San hô có tính tương hơp sinh học cao. Phẫu thuật giúp bệnh nhân cải thiện triệu chứng rõ đồng thời giảm hoặc mất cảm giác trống, rỗng trong hốc mũi.

Background: The Atrophic rhinitis is a relative uncommon disease in the world, but has appeared in Vietnam. Medical treatment prevents it temporarily, but if the treatment is not continued, the disease recurs. Over years of research, it is clearly that surgical treatment can help reduce the disease symptoms.

Methods: 4 patients who were diagnosed as the victims of the atrophic rhinitis and not cured by medical treatments, are hospitalized to operate for narrow noses. Corals are firstly transplanted on both sides of the nasal cavity of patients based on noses barometric. Next, corals are transplanted into the sub mucosal septum and floor of noses to reduce the volume of front and 2 sides of noses.

Results: Surgical treatment for 4 patients: three patients whose symptoms are improved significantly: no smell, no scales, no headaches, no nose bleeds..., and one patient whose symptoms are reduce clearly but not completely. No cases have occurred complications such as infection, transplant rejection or re-absorption of the materials.

Conclusions: we transplant corals into the sub mucosal septum and floor of noses under the endoscopic anesthesia tracheal is quite simple. The corals have high biological compatibility. Surgery helps patients improve their symptoms clearly and reduce or prevent the feeling of emptiness at the nasal cavity.
